It’s a common story—a patient is prescribed clobetasol cream to treat lichen sclerosus, but despite using the medication exactly as prescribed, no lasting relief is to be had.

What Is Clobetasol and Why Is It Prescribed First?
Clobetasol is a very strong topical steroid and is widely used as a first-line treatment for vulvar lichen sclerosus. It is prescribed first because it can reduce inflammation and ease itching and irritation.
These features make clobetasol an important tool, especially early on. But unfortunately, it is still primarily a tool for controlling symptoms and limiting damage. It is not designed to rebuild tissue that has already been affected, and for some patients, it becomes less helpful over time.
Why Clobetasol May Stop Working
There are a few reasons clobetasol may stop feeling like enough. In some cases, the tissue simply does not respond as expected. When skin does not respond to steroid creams, a biopsy may sometimes be recommended to rule out other causes. Even when treatment works well for a while, symptoms often come back, which is one reason long-term follow-up is so important in lichen sclerosus care.
There is also the larger issue of symptom management versus the condition itself. Lichen sclerosus is a chronic inflammatory disease, and while clobetasol can calm symptoms, that does not always mean the disease process has truly settled down. Long-term use can also raise concerns because prolonged topical corticosteroid use may contribute to further thinning of already fragile skin. If clobetasol is no longer giving you the relief it once did, that is not a personal failure. It may simply mean your body needs a different plan.
